Your Protected Health Information (PHI) includes personally identifiable details about your health, testing, and payment. Swift DNA is legally required to protect your PHI and provide this Notice outlining our privacy practices — including when and why we may use or disclose your information.

We will only use or disclose the minimum necessary PHI except as outlined here. This Notice may be updated from time to time; the latest version will always be posted at www.swift-dna.com. You may also request a printed copy by mail or during your appointment.

Uses & Disclosures That Do Not Require Your Consent

We may use or disclose your PHI without written authorization for:

Treatment: To coordinate care with healthcare providers and labs.

Payment: For billing and payment processing.

Healthcare Operations: Includes quality improvement, staff training, and appointment coordination.

As Required by Law: Such as court orders, law enforcement, or mandated reporting.

Death-Related Purposes: To coroners, funeral directors, or medical examiners.

Workers’ Compensation: As permitted by applicable laws.

Involvement in Your Care: To family or others involved in your care, including in emergencies or disaster relief efforts.

Uses & Disclosures That Require Written Consent

The following uses require your signed authorization, which you may revoke in writing at any time:

Marketing or Promotional Use (not applicable at Swift DNA unless you opt in).

Psychotherapy Notes (if applicable).

Research Participation (only if voluntary).

Any other disclosures not described in this Notice.

Your Rights Regarding PHI

You have the right to:

Access your PHI and obtain a copy (with limited exceptions).

Request corrections to your records.

Request restrictions on certain uses or disclosures (though we may not be required to agree).

Choose how we contact you (e.g., phone, email, mail).

Receive an accounting of disclosures made in the past six years (upon request).

Be notified of a breach involving your PHI.

Request a paper copy of this Notice at any time.

HIPAA Acknowledgment & Authorization for DNA Testing

By using Swift DNA’s services, you acknowledge that you have been offered access to this Notice and agree to the privacy practices described. No signature is required during booking or on our website.

Authorization for Use and Disclosure of Information

I authorize Swift DNA to:

Collect my buccal swab sample.

Use and disclose my PHI and genetic information as needed to complete DNA testing.

Share results with authorized labs, providers, and parties involved in the testing process.

Communicate results to me through secure and reasonable means.

This authorization remains valid unless revoked in writing. Any revocation does not affect disclosures already made.
